To ensure there are no account delays with your account set-up, please provide the required documentation for address verification. Proof of Residency ("POR") should be sent to the SHA Registrar as noted in the registration instructions.

Important: At any time a Member can be asked to provide additional Proof of Residency if deemed necessary by Southwest Hockey or Hockey Calgary.

Residency information must;

• Be in the name of the parent/guardian that the child resides with.
• The scan must CLEARLY show the FULL DATE (within the last month), Company name, your name and BOTH your mailing and service addresses.
  
• The scan must be clear and legible.

If you access your bills online, you can download a copy directly from your account. If necessary, you may also request a copy by email from your utility provider. The document MUST INCLUDE all required information listed above.
